In 2022, the Gulf Coast Partnership, Continuum of Care(CoC) Lead Agency on Homelessness for Charlotte County, was one of only four communities in Florida awarded the SAMHSA Grants for the Benefit of Homeless Individuals (GBHI). This five-year grant significantly strengthens our ability to address chronic homelessness among individuals and families in Charlotte County.
With this funding, the One Charlotte Integrated Care Team provides a wide range of essential services, including:
✔ Mental health and substance use outreach
✔ Peer support services
✔ SSI/SSDI Outreach, Access, and Recovery (SOAR)
✔ Mental health case management
✔ Therapy and counseling services
These services are coordinated through a partnership between the Gulf Coast Partnership and Charlotte Behavioral Health Care, ensuring that those in need receive holistic and integrated care. Over the life of the program, we aim to support 300 vulnerable individuals, including youth, Veterans, and families, who are experiencing homelessness and are affected by mental health challenges, substance use disorders, or both. This collaborative effort underscores our commitment to building a healthier, more stable community for all.
