The One Charlotte Coordinated Entry System is a centralized process designed to streamline access to housing and support services for Charlotte County residents experiencing homelessness or at risk of becoming homeless. This process ensures that each individual or family is assessed, prioritized, and connected to the most appropriate housing interventions based on their unique needs and circumstances.
How It Works:
The Coordinated Entry System follows a systematic approach:
1. Assessment: Residents in need complete a standardized assessment with a trained case manager, either in person or by phone at a participating Access Point.
2. Prioritization: Each assessment evaluates factors such as vulnerability, length of homelessness, and health needs to prioritize those in most urgent need.
3. Matching to Services: Based on priority and program availability, residents are referred to available housing solutions, such as emergency shelters, Rapid Re-Housing, Permanent Supportive Housing, and other community resources.
