Integrated Care, Lasting Impact

Over the past two years, the Gulf Coast Partnership (GCP), in collaboration with Charlotte Behavioral Health Care (CBHC), has transformed the lives of individuals experiencing homelessness and co-occurring disorders in Charlotte County through the One Charlotte Integrated Care Team (OCICT) and made possible by SAMHSA's Grant for the Benefit of Homeless Individuals.

A Model of Meaningful Change

Since its inception, the OCICT has served 120 individuals, helping 60 secure permanent housing, and improving the functional health of over 85% of those discharged from the program. But beyond these numbers are the real stories of resilience, recovery, and reintegration.

In just two years:

  • Social connection skyrocketed — In Year 1, only 40% of participants were socially connected at intake, yet by discharge, that number rose to 96%. In Year 2, the trend held strong: from 39.4% at intake to 93.9% at discharge.
  • Permanent housing retention has been maintained at an exceptional 93.61%.
  • 7 individuals had their SSI/SSDI benefits approved or reinstated, helping pave the way to greater stability.

Surpassing National Benchmarks

On nearly every National Outcome Measure (NOM), Charlotte County’s GBHI program is outperforming national averages—especially in the most meaningful areas of transformation:

  • Social Connectedness improved from 68.2% to 95.5%, compared to a national shift of just 0.9%.
  • Employment/Education Participation increased from 13.6% to 40.9%—a 27.3% jump, significantly outpacing the national average.
  • While abstinence rates are still an ongoing challenge, the program’s focused support services are laying the groundwork for long-term recovery.

Voices of Transformation

Client satisfaction surveys provide an even deeper glimpse into impact. In a sample of individuals tracked over six months and again at discharge, results showed a remarkable upward trend:

  • I like the services I received” saw high levels of agreement, validating the program’s client-centered approach.
  • I am better able to control my life” and “My housing situation has improved” were among the most positively endorsed statements.

These outcomes show that the One Charlotte Integrated Care Team isn’t just meeting benchmarks—it’s reshaping what’s possible for some of our community’s most vulnerable residents.
