Over the past two years, the Gulf Coast Partnership (GCP), in collaboration with Charlotte Behavioral Health Care (CBHC), has transformed the lives of individuals experiencing homelessness and co-occurring disorders in Charlotte County through the One Charlotte Integrated Care Team (OCICT) and made possible by SAMHSA's Grant for the Benefit of Homeless Individuals.
Since its inception, the OCICT has served 120 individuals, helping 60 secure permanent housing, and improving the functional health of over 85% of those discharged from the program. But beyond these numbers are the real stories of resilience, recovery, and reintegration.
