The Food Connection was a 3-year project created with the mission, To alleviate hunger by improving the systems and services of food delivery in Charlotte County. Year 3 Food Connections partners were:

- Holy Trinity Lutheran Church
- The Homeless Coalition
- Human Services Department
- Port Charlotte Seventh-day Adventist Church
- Pilgrim Church
- St. Francis of Assisi
- St. Vincent de Paul – Sacred Heart
- St. Vincent de Paul – San Antonio
- St. Vincent de Paul – St. Charles Borromeo
- St. Vincent de Paul – St. Maximilian Kolbe Conference
- Trinity United Methodist Church
The goals of year 3 included:
- Training more case managers
- Expanding Getting Ahead classes
- Increasing case management services
- Decreasing high-end food pantry utilizers
Year 3 accomplished these goals by increasing the number case management services from 841 to 1066, an increase of almost 27%, expanding Bridges Out of Poverty training for case managers, and decreasing high-end utilizers from 261 in year 1 to only 25 in year 3.
